Ultherapy is a brand of focused-ultrasound lifting device made by Merz; HIFU is the broader category it belongs to. In Tokyo, clinic websites list full-face Ultherapy at about ¥190,000 to ¥440,000, while a full-face session on other HIFU devices (ULTRAcel, Ultraformer, New Doublo and others) is listed at about ¥59,000 to ¥200,000. Most Tokyo clinics that offer Ultherapy have Japanese-only websites; Matsurika Clinic (Ginza) and Tokyo Midtown Skin Aesthetic Clinic Noage (Roppongi) state that English is available. Clinics whose doctors offer consultations in English, such as Omotesando Rejuva Medical Clinic and Art Lounge Clinic, offer other HIFU devices rather than Ultherapy.
HIFU (high-intensity focused ultrasound) is the category: any device that focuses ultrasound energy at a set depth beneath the skin to heat and tighten tissue. Ultherapy is one product in that category, made by Merz, and it was the first ultrasound lifting device cleared by the US FDA. Japanese clinics refer to it as Ultherapy, Ulthera or ウルセラ; the newer generation is sold as Ultherapy PRIME (ウルセラピープライム).
The practical difference is imaging. The Ultherapy handpiece shows the physician an ultrasound image of the skin layers during treatment, so the energy can be placed at the intended depth, usually 3.0 mm and 4.5 mm. Other HIFU devices used in Tokyo (ULTRAcel Zi and Q+, Ultraformer III, Doublo Gold and New Doublo, Liftera and others) treat at fixed depths without a live image. They cost much less per session, and they are what most Tokyo clinics mean when a menu simply says "HIFU."
Some clinics give their protocol a name of its own: Aoi Clinic Ginza calls its Ultherapy treatment "Ultherapy Cross Millefeuille" (ウルセラ・クロスミルフィーユ), and Tokyo Biyo Hifuka Clinic lists separate prices for Ultherapy and Ultherapy PRIME performed by a certified doctor. The underlying device is still Ultherapy.
Among the clinics in this guide, full-face Ultherapy ranges from ¥190,000 (Tokyo Isea Clinic, 500 shots, first visit) to ¥440,000 (Ginza Takasu Clinic). Adding the area under the chin or the neck widens the range to ¥220,000 (Tokyo Isea, 550 shots) up to ¥660,000 (Ginza Takasu, full face plus under the chin to the upper neck). Matsurika Clinic prices by shot count, from ¥252,560 for 348 shots to ¥428,000 for 760 shots on the full face.
A full-face session on another HIFU device ranges from ¥59,400 (Ledian Clinic, ULTRAcel Zi, 600 shots) to ¥199,600 (Art Lounge Clinic, New Doublo Premium plan); Omotesando Rejuva Medical Clinic lists its ULTRAcel Q+ full face at ¥132,000. In other words, one Ultherapy session costs roughly as much as two to four full-face sessions on another device.
Repeat pricing is common for Ultherapy: Tokyo Isea Clinic lists a lower price for a repeat session within one year, and Matsurika Clinic charges ¥11,000 for an Ultherapy consultation on its own. Anesthetic cream is extra at most clinics (¥5,500 at Tokyo Biyo Hifuka, which also lists a local anesthetic injection at ¥22,000). All prices in this guide include tax unless noted.
Clinic websites present Ultherapy as the option for someone who wants the imaging-guided, higher-energy treatment and is prepared to pay for it, usually once a year or less often, and other HIFU devices as the option for a smaller budget or for lighter, more frequent sessions. Tokyo Isea Clinic warns that Ultherapy can feel like intense burning heat during treatment; Ginza Takasu Clinic gives 40 to 60 minutes as the treatment time. Art Lounge Clinic gives four to six months as the usual interval between New Doublo sessions.
Neither treatment is surgery, and neither replaces a facelift. If you are comparing the two, the questions that change the answer are these: which depths will be treated, how many shots or lines are included for the area you want, who performs the treatment (a certified doctor, a doctor or a nurse), what a repeat session costs, and whether an English consultation can be booked.
Of the Ultherapy providers in this guide, Matsurika Clinic (Ginza) states "English available" and publishes its Ultherapy prices in English; Tokyo Midtown Skin Aesthetic Clinic Noage (Roppongi) states that English-speaking staff are available and publishes an English price list; and Ginza Takasu Clinic says it has many English and Chinese interpreters on staff. Aoi Clinic Ginza, Tokyo Isea Clinic, Shirono Clinic and Tokyo Biyo Hifuka Clinic publish in Japanese only.
If an English-speaking doctor matters more to you than the Ultherapy brand, two clinics in this guide have doctors who offer consultations in English and offer other HIFU devices: Omotesando Rejuva Medical Clinic (ULTRAcel Q+; three doctors offer consultations in English) and Art Lounge Clinic (New Doublo; the director offers consultations in English by appointment). Art Lounge Clinic states plainly that it offers New Doublo, not Ultherapy, and that the two are different ultrasound platforms.
